Frequently Asked Questions

Q: Do the medical plans use Preferred Provider (PPO) networks?
A: Yes all the group medical plans utilize the PHCS Preferred Provider Network to reduce your healthcare costs and maximize plan benefits.

Q: Is there a “co-pay” for doctor visits?
A: No. There are no co-pays for doctor visits under any of the medical plans.

Q: What about the Dental Plans - are networks involved?
A: No. You are free to choose any dentist you like. There are no required networks or co-pays in the Benefit Alliance Dental Plan.

Q: May I cover my family members?
A: Yes. You may cover eligible family members in all Benefit Alliance Plans with the exception of the Short Term Disability Income Plan.

Q: Can I enroll my domestic partner?
A: You may enroll a domestic partner into any plan provided that you meet the following criteria.

"Domestic Partner" means a person of the same or opposite sex of the Insured who:
1) shares the Insured's primary residence.
2) has resided with the Insured for at least 12 months prior to the date of enrollment and is expected to reside with the Insured indefinitely.
3) is financially interdependent with the Insured in each of the following ways;
a. by holding one or more credit or bank accounts, including a checking account, as joint owners.
b. by owning or leasing their permanent residence as joint tenants;
c. by naming, or being named by the other as a beneficiary of life insurance or under a will.
d. by each agreeing in writing to assume financial responsibility for the welfare of the other.
4) has signed a Domestic Partner declaration with Insured, if recognized by the laws of the state in which he or she resides with the Insured.
5) has not signed a Domestic Partner declaration with any other person within the last 12 months.
6) is 18 years of age or older.
7) is not currently married to another person.
8) is not in a position as a blood relative that would prohibit marriage.

 

Q: Can I enroll in only one benefit or must I select all of them?
A: You may select any combination of one or more the the benefit plans offered.

Q: Is the Benefit Alliance Plan payroll deducted by my employer?
A: No. The monthly premium for all the Benefit Alliance Plans you select will be combined into a single convenient monthly payment. You can choose to receive a monthly bill or pay by automatic bank draft.

Q: When do my benefits go into effect?
A
: If Leslie & Associates receives all of the required enrollment materials from you by the 20th of the month, your coverage will become effective the first day of the following month."Required enrollment materials" means a correct and completed Enrollment Form and a check or money order for the first month's premium for all plans selected including the initial enroll ment and administrative fees.

Example: All required enrollment materials received by Leslie & Associates by October 20th - coverage will become effective November 1st.

Example: All required enrollment materials received by Leslie & Associates after October 20th but before November 1st - coverage will become effective December 1st.

Q: Can I make changes to my policies at any time?
A: It depends on the benefit. You can always make changes within 30 days of the effective date or durring the open enrollment period.. You can also make changes to policies after a "Life Changing Events" i.e. marriage or birth of a child. Medical plans can only be changed durring the month of December.

Q: How do I cancel one or more of my selected plans?
A: In order to cancel one or more of your plans (you do not have to cancel all of your plans in order to eliminate ones you may no longer need), you must notify Leslie & Associates in writing by mail or fax. Policies cannot be verbally cancelled over the telephone. Since these benefits are group policies, cancellations will become effective on the last day of the month in which you requested cancellation.

Q: What happens when I leave Kelly or I'm between assignments?
A: You can keep your plans as long as you continue to pay your monthly premiums as scheduled at the same rate. The only plans you will not be able to keep are Life Insurance (which can be ported or converted) and/or the Short Term Disablity plan.
