Frequently Asked Questions

Q: Do the medical plans use Preferred Proivder (PPO) networks?
A:Some of the Benefit Alliance medical plans utilize a PPO network and some do not. Please read the individual plan descriptions for more detailed information.

Q: Is there a “co-pay” for doctor visits?
A: Again, it depends on the plan design you choose. Refer to the plan descriptions.

Q: What about the Dental Plans - are networks involved?
A:No. You are free to choose any dentist you like. There are no required networks or co-pays in the Benefit Alliance Dental Plan.

Q: May I cover my family members?
A:Yes. You may cover eligible family members in all Benefit Alliance Plans with the exception of the Short Term Disability Income Plan.

Q: Can I enroll in only one benefit or must I select all of them?
A:You may select any combination of one or more the the benefit plans offered.

Q: Is the Benefit Alliance Plan payroll deducted by my employer?
A:No. The monthly premium for all the Benefit Alliance Plans you select will be combined into a single convenient monthly payment. You can choose to receive a monthly bill or pay by automatic bank draft.

Q: When do my benefits go into effect?
A
:If Leslie & Associates receives all of the required enrollment materials from you by the 20th of the month, your coverage will become effective the first day of the following month."Required enrollment materials" means a correct and completed Enrollment Form and a check or money order for the first month's premium for all plans selected including the initial enroll ment and administrative fees.

Example: All required enrollment materials received by Leslie & Associates by October 20th - coverage will become effective November 1st.

Example: All required enrollment materials received by Leslie & Associates after October 20th but before November 1st - coverage will become effective December 1st.

Q: Can I make changes to my policies at any time?
A:It depends on the benefit. You can always make changes within 30 days of the effective date during an "Open Enrollment" period or within 60 days from the first day of a new assignment. You can also make changes to policies after a "Life Changing Events" i.e. marriage or birth of a child.

Q: How do I cancel one or more of my selected plan?
A:In order to cancel one or more of your plans (you do not have to cancel all of your plans in order to eliminate ones you may no longer need), you must notify Leslie & Associates in writing by mail or fax. Policies cannot be verbally cancelled over the telephone. Since these benefits are group policies, cancellations will become effective on the last day of the month in which you requested cancellation.

Q: How do I cancel one or more of my selected plans?
A: In order to cancel one or more of your plans (you do not have to cancel all of your plans in order to eliminate only one or two that you may no longer need), you must notify Leslie & Associates by mail or fax. Policies cannot be canceled verbally over the telephone. Policy cancellations will become effective on the last day of the month during which you requested cancellation.
